What you will fill in

- Who is giving notice?
- Owner's full name
- Renter's full name
- Address and room
- Move-out date
- Date of this document
Good to know
- Check the first question carefully: if the renter does NOT share a kitchen or bathroom with the owner or close family living there, this notice is not valid in Ontario. Use the LTB guide on this site instead.
- Give reasonable notice and keep a copy. Never change locks or remove belongings while the renter is still entitled to stay; if there is a dispute, get legal advice.
- If you signed a fixed-term agreement, follow what it says about ending early.
Help in your language
Every question comes with help like this. For example, “Does the renter share the kitchen or bathroom with the owner (or the owner's spouse, child or parent) who lives there?”:
English: If not, Ontario's Residential Tenancies Act applies and you must use the Landlord and Tenant Board's N-forms instead.
