What you will fill in
- Address of the home you are applying for
- Your full name
- Phone
- Current address
- How long have you lived there?
- How many people will live there?
- Move-in date you want
- Do you agree to a credit check?
Good to know
- This form does not ask about things Ontario's Human Rights Code protects, like age, religion, place of origin or family status.
- Never write your Social Insurance Number (SIN) on a rental application. Landlords don't need it.
