What you will fill in
- Which province is the rental home in?
- Landlord's full legal name
- Tenant's full name (all tenants on the lease)
- Address of the rental unit
- Total rent owing ($)
- Which rent periods are unpaid?
- Date you will serve the notice
- How will you serve it?
- Date you sign the notice
Good to know
- Alberta: the end date must be at least 14 days after service. Count the day of service carefully and add a day or two to be safe.
- A notice is not an eviction. Only a tribunal or court order, enforced by a sheriff or civil enforcement agency, can remove a tenant. Never change the locks or remove belongings yourself.
- Accepting rent after the notice can cancel it in some provinces. If you accept partial payment, say in writing that the notice still stands, where the law allows.
