What you will fill in
- Does any of these apply to you?
- Your full legal name
- Your city or town, in British Columbia
- Life-support and life-prolonging treatment if there is no reasonable hope of recovery
- CPR (restarting the heart) if your heart stops
- Person to call (name and phone)
- How will you have it witnessed?
Good to know
- An advance directive speaks for you directly to your health care providers. A Representation Agreement instead names a person to decide. You can have both.
- If you have a representative, your representative must follow the instructions in this advance directive.
- Give copies to your doctor, your hospital and your family. Review it every few years or when your health changes.
Help in your language
Every question comes with help like this. For example, “Does any of these apply to you?”:
English: If one does, a lawyer should check it before you sign. You can still make the full document here.
