What you will fill in
- Your full legal name
- Your city
- Province
- Who is it for?
- What facts are you declaring?
Good to know
- Do NOT sign it yet. Sign in front of a commissioner for taking affidavits, a notary public or a lawyer (many ServiceOntario and MP offices do this for free).
- Many federal offices have their own declaration form (for example IRCC's common-law form): use theirs if they give you one.
- Making a false statutory declaration is a crime.
