- Employer's legal name and address
- What does your business do?
- How many employees do you have in Ontario?
- Normal work hours
- How often is pay day?
- Who do employees report problems to?
- Second person to report to
- Do you monitor employees electronically?
- How and why do you monitor?

Good to know
- Ontario requires you to post the 'Employment Standards in Ontario' poster and give each employee the ESA information sheet. Download it at ontario.ca.
- Policies can give more than the ESA but never less. A rule that gives less is void.
- With 25 or more employees, give a copy of the disconnecting and monitoring policies to each employee within 30 days of making or changing them.
